A woman stares in the mirror as she applies lip liner to her newly injected lips.

The pencil on your vanity and the syringe in the treatment room both promise the same thing. Here’s what sets them apart.

You’ve seen it. The viral TikTok videos with 4 million saves showing exactly how to overline your lips with a fresh-toned pencil, load on a plumping gloss, and walk out the door looking like you woke up with a fuller pout. It works… and it doesn’t. That’s the honest truth not always said out loud.

Makeup and medicine occupy two very different categories when it comes to your lips. Both are valid. Both have a time, a place, and a person they’re right for. The question isn’t which one is better; it’s which one is right for you, right now.

At Healthy Complexions Spa, we believe in giving you real information so you can make real decisions. No hard sell. No pressure. Just what you actually need to know.

The Art of the Overline & It’s Honest Limits

Overlining is a legitimate, skilled makeup technique. When done well — with the right products, the right hand, and the right lighting — it can create the illusion of a slightly fuller lip border.

But here’s what overlining can’t do:

It cannot add actual volume. It cannot address the asymmetry between your upper and lower lip. It can’t restore the projection and fullness that comes with age-related volume loss. It peaks. And the moment the light changes, you turn to the side, or the day wears on, the illusion shifts.

None of that is a criticism of the technique. It’s simply the nature of a 2D solution applied to a 3D problem.

Overlining

  • Creates the illusion of fuller borders
  • Zero downtime, fully reversible
  • Washes off at the end of the day
  • Doesn’t add actual volume
  • Cannot correct asymmetry or shape
  • Results vary depending on skill level

Lip Filler

  • Adds real, three-dimensional volume
  • Corrects asymmetry & defines shape
  • Lasts 6-12+ months
  • Softens fine lines around the mouth
  • Results are consistent
  • Reversible with hyaluronidase

What Lip Filler Actually Does For You

Hyaluronic acid (HA) filler injected into the lips does something no makeup product can: it physically adds structure. The HA gel integrates with your tissue, creating real projection, real definition, and real fullness that you can feel as much as you see.

At Healthy Complexions Spa — the only med spa in Western New York under the direction of a board-certified dermatologist — your treatment starts with a conversation. What do you like about your lips? What bothers you? What do you actually want to change? That conversation shapes everything about your results.

Different products do different things. Juvederm adds soft, plush volume. Revance RHA moves more naturally with the lips and is excellent for subtle definition. Redensity works beautifully on fine lip lines. Your injector builds a custom plan around your anatomy, not a trend.

Shape, Symmetry, and What Changes a Lip

Let’s talk honestly about symmetry; it’s where makeup and medicine have the starkest divide. Most people have lips that aren’t perfectly symmetrical. One side may sit higher. The upper lip may be noticeably thinner than the lower. The cupid’s bow might be undefined or lopsided. Overlining can work with symmetry; it can’t create it.

Filler can. With precise placement, an experienced injector can add volume to the side that needs it, sharpen the peaks of the cupid’s bow, and create balance that your lip liner doesn’t have the tools to achieve.

Similarly, as we age, lips thin and the borders soften. The crisp line between lip and skin begins to blur. Lip liner applied there addresses the appearance. Filler addresses the underlying cause.

So, Which Option is Right For You?

Here’s a simple way to think about it. Match your scenario below:

First Time at Healthy Complexions Spa? Here’s What to Expect

The number one thing first-timers tell us after their appointment: it was way easier than I thought.

We apply numbing cream before anything else. Most fillers used in lip treatment also contain lidocaine, which further reduces sensitivity during the procedure. The whole appointment is typically 30 to 45 minutes. Swelling is normal for the first 24 to 72 hours, and final results show up once everything settles.

Your consultation at Healthy Complexions Spa is free. It’s a conversation, not a sales pitch. We want to understand your goals and your anatomy before we ever pick up a syringe. If we think overlining or a great gloss will get you where you want to go, we’ll tell you that too.
